就職内定・進学決定届の入力について

進学、就職などの進路が決まったら、大学基幹業務ICTシステム（Live Campus U）の『就職内定・進学決定届』を入力いただくようご協力をお願いします。
『就職内定・進学決定届』により知り得た情報は、就職支援、進路状況の把握、就職実績の集計・分析、基礎資料の作成・発行、学外機関からの調査回答、文部科学省が行う統計法に基づく「学校基本調査」を含む法令・行政上必要な報告のために利用します。また、この情報は、学生支援及び本学教職員や学内の各部署の職務遂行上必要な範囲において、共有することがあります。
当該情報の利用・学内共有の際は、本学個人情報保護方針を遵守し、上記の目的以外には利用いたしません。併せて、学外へ氏名は一切公表しません。
また、学内の教職員、関係部署へ情報共有する際は、利用目的の把握を含め必要な手続きを行った上で、必要最小限の情報に限定する等、適切な情報管理の徹底に努めています。

The university requires students who have decided on their future plans-such as continuing their education or entering the workforce-to complete the “Employment and Further Education Decision Form” on Live Campus U.
The information obtained from the “Employment and Further Education Decision Form” will be used for employment support, tracking students’ career paths, compiling and analyzing employment status, creating and publishing reference materials, responding to surveys from external organizations, and fulfilling legal and administrative reporting requirements-including the “Basic Survey of Schools” conducted by the Ministry of Education, Culture, Sports, Science and Technology (MEXT) in accordance with the Statistics Act. In addition, this information may be shared to the extent necessary for student support and for the performance of duties by the University’s faculty, staff, and various internal departments.
When using or providing this information, we will comply with the University’s “Personal Information Protection Policy” and will not use it for any purposes other than those specified above. Furthermore, we will not disclose students’ names to external parties.
Furthermore, when sharing information with faculty, staff, and relevant departments within the university, we will strive to ensure appropriate information management by adhering to necessary procedures-including confirmation of the purpose of use-and limiting the information to the minimum necessary.
